Listening to his or her concerns, symptoms, and history is an important starting point in developing a diagnosis. The diagnosis is thoroughly discussed with the patient and I will answer any questions that present. Following the diagnosis, a plan of care is developed with the patients input. Patient preferences for therapy, medication, or both, are included in the treatment plan, and any referrals necessary will be made. Medication, if necessary and agreed upon, will be called in to the patients preferred pharmacy and any remaining questions or concerns will be addressed. A follow-up visit to assess ongoing effectiveness of the treatment plan will be made. At consecutive appointments, any necessary changes to the treatment plan will be made. However, I am available for concerns, questions, or needs that may arise prior to follow-up visits when needed.
